Pre-K and Kindergarten Registration Packet (Please print single-sided)
♦ If your child is already receiving services through the district and has previously filled out a registration packet, a second registration packet will be required to enroll them in our Pre-K program. For any questions, please contact Sandy Friday.
Current Marion UPK students are already registered for Kindergarten. If your student did not attend UPK in Marion, please complete the registration process.

1. Parent or Legal Guardian
A parent or court-appointed guardian can enroll a student.
2. Caregiver or Relative (Person in Parental Relation)
Someone who has custody or control of the child (like a relative or caregiver) can enroll them. You may need to show proof of custody and responsibility.
3. Emancipated Students
If a student is legally emancipated, they can enroll themselves.

- Proof of Age: Birth certificate, passport, or other official documents
- Proof of Residency: Utility bill, lease agreement, or similar proof
- Immunization Records: Evidence of required vaccinations
- School Records: If transferring, records from the previous school
- Completed Registration Packet: Registration Packet (please print single-sided)

What other ways can I show proof of age?
If you don't have a birth certificate, baptismal record, or passport, you can use other documents if you've had them for at least two years, such as:
- Driver's license
- State or governmental ID
- School photo ID with date of birth
- Consulate ID card
- Hospital or health records
- Military-dependent ID card
- Other documents from federal/state/local agencies (examples: Department of Social Services, Office of Refugee Resettlement)
- Court orders
- Native American tribal document
- Records from international aid agencies or voluntary agencies
What other documents can I use for proof of residency?
Ways you can show residency are as follows:
- Lease or deed
- Affidavit from the person you pay rent to, stating that you live there.
- A letter from the person you pay rent to, stating you live there.
- Owner Statement of Residency (link to form can be found below) - A notarized document stating you live with another person who resides within the district.
- Other documentation, such as:
- Pay stub showing your address
- Utility bill or another bill in your name
- Membership documents based on residency, such as a local library card
- Voter registration card
- Driver's license, or permit, or non-driver ID
- State or other government-issued ID
- Documents from government agencies such as social service agencies or the federal Office of Refugee Resettlement
- Custody or guardianship papers
